Referrals & Admissions

Survivors of traumatic brain injury or neurologic injury and individuals with neuropsychiatric disorders are referred on a national level. Patients are individually reviewed and qualified for admission on a clinical, medical and financial basis.

Family members and other concerned parties are encouraged to tour the program prior to admission in order to make informed placement decisions. Oppidan assists in lessening the financial burden placed on family members by absorbing some of the costs related with travel expenses, meals, and overnight accommodations.

Our qualified representatives are available nationally to conduct pre-screening field evaluations or schedule facility tours at no cost or obligation. Admission Criteria

Survivors of traumatic brain injury or neurologic injury and individuals with neuropsychiatric disorders are referred on a national level. Patients are qualified for admission individually on a clinical, medical and financial basis.

Family members and other concerned parties are encouraged to tour the program prior to admission to make informed placement decisions. Oppidan assists in lessening the financial burden placed on family members by absorbing some of the costs associated with travel expenses, meals, and overnight accommodations.

All referrals for rehabilitation are reviewed on an individual basis. The criteria listed below are general guidelines and do not individually qualify or disqualify eligibility.

1. Documented brain injury or other neurologic disorder.

    Such as:

  • Traumatic Brain Injury
  • Neurologic Injury/Disorder
  • Central Nervous System Injury
    As a result of:

  • Closed or penetrating head injury
  • Central nervous system disease
  • Brain tumors
  • Cerebrovascular accidents
  • Exposure to neurotoxin
  • Metabolic disorders
  • Anoxic/hypoxic encephalopathies
  • Varied encephalopathies

2. Eighteen years of age or older.

3. Achieved a stable level of medical recovery

4. Ability to participate

The Referral Process

1. Anyone can initiate a referral.

  • Examples include Clients, their families, physicians, social workers, discharge planners, insurance case managers, guardians, community agencies, attorneys, or anyone else seeking placement options

2.Information Needed to start a referral:

  • Family contact, guardian, and/or decision maker
  • Client information including: age, injury information, and current location.

3.Referrals can be made by:

Our knowledgeable representatives are always available to meet the needs of families, funders, and referral sources. In most cases, response time is less than 24 hours from receipt of the referral. If the need for an urgent response is necessary, staff can be on-site the same day in most cases.

The Admission Process

1. An intake and Referral Form is completed.

2. An Admissions Coordinator is assigned as the primary contact throughout the entire process.

3. A pre-admission screening is scheduled without cost or obligation.

4. A release of information is obtained from the potential client or their legal guardian.

5. Pre- and post-injury medical and other records are obtained.

6. Review of the pre-admission screening and available medical records by the clinical neuropsychologist to determine medical/clinical program eligibility.

7. An individualized Proposed Treatment Plan is developed with outlines the potential client’s goals and objectives for admission.

8. At the convenience of the concerned parties, a facility tour is scheduled which addresses the specific rehabilitation and care needs of the potential client. The touring party will have the opportunity to meet with and ask questions of the clinical staff.

9. Letters of Medical Necessity are obtained from the client’s treating physicians.

10. The Proposed Treatment Plan is submitted for family review and approval, medical precertification, and other payor source review and approval as appropriate.
